Our office is happy to assist patients and families with forms that require completion by our medical team, including school medication forms, camp forms, and other healthcare related documentation.
Please allow up to two (2) weeks for our office to review and complete requested forms. Forms may require review of the patient's medical record and, when appropriate, consultation with the healthcare provider.
To help us process your request efficiently:
- Patient must have an annual visit with our office in order for school forms to be completed. If your child has not been seen within the past year, an annual appointment must be scheduled and the child seen before our office can complete forms.
- Plan ahead and submit forms well before school, camp, or other deadlines.
- Urgent or last-minute requests may not be able to be accommodated.
We appreciate your understanding and cooperation. The two-week processing time helps ensure that forms are completed accurately, thoroughly, and with appropriate medical review.
